Modern Senior Woman

Whose Eligible

Individuals who receive Medicaid benefits, Veterans Benefits, or Private Pay AND:

​

  • Are 5 years or older(EPSDT, Private)

  • Are 21 Years or older (All Others)

  • Meet nursing facility level of care

  • At minimum require limited assistance with one or more daily living activities

  • Are able to direct their care themselves or through a responsible representative

  • Are in a nursing facility and is able to be discharged if community-based services are available

  • Are likely to require nursing facility admission within the next 120 days.
